Welcome to Serene Wellness Oasis MedSpa (“we,” “our,” or “us”). By accessing or using our website,
services, online booking tools, forms, or communications (collectively, the “Site”), you agree to be bound by
these Terms of Service (“Terms”). If you do not agree, please do not use the Site.

1. Use of Website
The Site is intended for individuals who are at least 18 years old. By using the Site, you confirm you meet
this requirement. You agree to use the Site only for lawful purposes and in compliance with all applicable
laws and regulations.

2. No Medical Advice Disclaimer
Content on the Site—including text, images, educational materials, and blog content—is provided for
informational purposes only and is not medical advice. It is not a substitute for professional medical
evaluation, diagnosis, or treatment. Do not disregard medical advice or delay seeking care because of
content on the Site.

3. Medical Services, Eligibility, and Provider Approval
Some services offered by Serene Wellness Oasis MedSpa are medical in nature and require medical
screening and provider approval. All services are subject to:
• completion of intake documentation and medical history review
• a Good Faith Exam (GFE) when required by applicable law and standard of care
• medical appropriateness as determined by a licensed provider
We reserve the right to refuse, postpone, or discontinue services if they are deemed inappropriate, unsafe,
or not in the client’s best interest.

4. Appointments, Late Arrivals, Cancellations & No-Shows
We value your time and ours. Appointment policies may vary by service and will be disclosed at booking;
however, unless otherwise stated in writing:
• Cancellations/reschedules must be made at least 48 hours prior to the appointment time.
• Late cancellations and no-shows may result in forfeiture of any deposit and/or a fee.
• Late arrivals may require shortening the service time or rescheduling, and may be treated as a late
cancellation depending on timing and scheduling constraints.
5. Payments, Deposits, and Refund Policy
• Prices are subject to change at any time and may vary based on clinical assessment and product usage.
• Deposits (if applicable) are generally non-refundable and may be applied toward the scheduled service
when the appointment is kept as scheduled.
• All services are final once rendered. We do not issue refunds for services already performed.
• We do not provide refunds for injectable products once prepared, dispensed, or administered, or for
products that cannot be returned due to safety/sterility reasons.

6. Packages, Promotions, and Gift Cards

• Packages and promotional pricing may have expiration dates and specific terms disclosed at purchase.

• Packages are non-transferable unless stated otherwise in writing.
• Gift cards are non-refundable and may be subject to additional terms and restrictions as allowed by law.

7. Results Disclaimer

Individual results vary and depend on many factors including anatomy, lifestyle, metabolism, and adherence
to aftercare instructions. We do not guarantee specific outcomes or longevity of results.

8. Intellectual Property
All content on the Site (including logos, graphics, copy, images, and branding) is owned by Serene Wellness
Oasis MedSpa or used with permission and is protected by intellectual property laws. You may not copy,
reproduce, distribute, modify, or use content without prior written consent.

9. Third-Party Links and Services
The Site may contain links to third-party websites, booking platforms, payment processors, or financing
services. We are not responsible for third-party content, policies, or practices. Your use of third-party
services is governed by their terms and policies.

10. Electronic Communications
By using the Site and providing your contact information, you consent to receive communications from us
electronically (e.g., email, SMS) regarding appointments, services, promotions (where permitted), and
operational messages. Message and data rates may apply. You may opt out of marketing messages as
required by law.

11. Limitation of Liability
To the fullest extent permitted by law, Serene Wellness Oasis MedSpa is not liable for any indirect,
incidental, special, consequential, or punitive damages arising from your use of the Site or services, or
reliance on Site content. Our total liability for any claim shall not exceed the amount paid to us for the
specific service giving rise to the claim, to the extent permitted by law.

12. Indemnification
You agree to indemnify and hold harmless Serene Wellness Oasis MedSpa and its owners, employees,
contractors, and affiliated providers from any claims, liabilities, damages, and expenses arising out of your
misuse of the Site, violation of these Terms, or violation of any law or third-party rights.

13. Governing Law
These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.

14. Changes to Terms
We may update these Terms at any time. The “Last Updated” date will reflect the latest revision. Continued
use of the Site after changes constitutes acceptance of the updated Terms.

GLP-1 WEIGHT LOSS PROGRAM & MEMBERSHIP TERMS OF SERVICE

These terms (“GLP-1 Terms”) apply to all medically managed weight loss programs, memberships,
subscriptions, and related services offered by Serene Wellness Oasis MedSpa, including but not limited to
GLP-1 medications such as semaglutide and tirzepatide, adjunct injectable therapies (e.g., B12, MIC,
Lipo-C, NAD+, vitamin therapies), and coaching/education (collectively, the “Program”). By enrolling in the
Program, you agree to these GLP-1 Terms in addition to our general Terms of Service and Privacy/HIPAA
notices.

1. Medical Nature of Program
The Program involves medical decision-making and is not a guarantee of weight loss. Participation requires
medical screening and provider approval, which may include:
• medical intake forms and history review
• review of current medications and contraindications
• baseline vitals and/or labs (as clinically indicated)
• a Good Faith Exam (GFE) when required by applicable law and standard of care
We may decline or discontinue enrollment if the Program is not medically appropriate or safe.

2. No Guarantee of Results
Weight loss outcomes vary. We do not guarantee a specific amount or rate of weight loss, improvements in
body composition, or long-term maintenance. Results depend on individual physiology, adherence, lifestyle,
and medical factors.

3. Memberships, Inclusions, and Changes
Memberships may include (depending on your selected plan): provider-led medication management,
check-ins, education, supplies, and/or discounted services. Plan inclusions, frequency of check-ins, and any
limits are disclosed at enrollment and may change periodically. We will provide notice of material changes
as required.

4. Billing, Payments, Renewals
• Membership fees are billed on a recurring basis (e.g., monthly) unless you select a different term.
• Memberships may auto-renew unless canceled under Section 5.
• Payments are due in advance of service periods. Failed or reversed payments may result in paused
services, delayed refills, or program removal.
• Prices are subject to change with advance notice.

5. Cancellation Policy
To cancel your membership/subscription, you must submit written cancellation notice at least 14 days prior
to your next billing date. Cancellations received after this window may take effect the following billing cycle.
We do not offer partial refunds or prorations for unused time, missed check-ins, or unused included benefits.
Medication already ordered, compounded, dispensed, or shipped is not refundable.

6. Medication Ordering, Dispensing, and Refill Timing

• Medications are dispensed only after provider approval and receipt of required information (e.g., weight,
vitals, symptom updates, pregnancy status when applicable).
• Refill timing depends on clinical appropriateness, adherence, and safety. We may require follow-up prior to
refills.
• We are not responsible for delays caused by supply chain issues, shipping delays, third-party pharmacies,
weather, or incomplete client information.

7. Medication Use, Storage, and Non-Transferability
• Medications are prescribed for the individual client only and may not be shared or transferred.
• You agree to follow dosing, administration, and storage instructions. Improper storage (temperature
excursions), contamination risk, or misuse may render medication unsafe and will not be replaced.
• Lost, stolen, damaged, or improperly stored medications are not eligible for replacement or refund.

8. Dosage Adjustments and Clinical Judgment
Dosage initiation, titration, maintenance, pauses, and discontinuation are determined by the licensed
provider based on clinical judgment, side effects, contraindications, and safety. You acknowledge that you
may be maintained, titrated more slowly, or discontinued based on symptoms or risk factors. You agree not
to self-adjust dosing without provider guidance.

9. Client Responsibilities
You agree to:
• provide accurate health information and promptly update changes (new diagnoses, meds, pregnancy,
allergies, surgeries, etc.)
• report side effects or adverse events promptly, including severe or persistent symptoms
• attend required check-ins (virtual or in-person) as clinically indicated
• follow nutrition, hydration, activity, and aftercare guidance provided for safety and effectiveness
Noncompliance may result in dose holds, refill delays, or program termination without refund.

10. Side Effects & Risks
You acknowledge that GLP-1 medications may cause side effects, including nausea, vomiting, diarrhea,
constipation, reflux, abdominal discomfort, appetite changes, fatigue, dizziness, and dehydration. Serious
risks and contraindications will be reviewed during your consultation/informed consent. Seek urgent care for
severe symptoms (e.g., persistent vomiting, severe abdominal pain, signs of allergic reaction, or
dehydration).

11. Telehealth & Electronic Communication
Parts of the Program may be delivered via telehealth. You consent to virtual consultations and electronic
communications for coordination of care. You understand there are privacy and security risks inherent in
electronic communication despite safeguards.

12. Self-Pay / No Insurance Billing
Unless expressly stated in writing, Program services are self-pay. We do not bill insurance, Medicare, or
Medicaid for membership services or compounded medications.

13. Program Pauses, Holds, and Termination
We may pause, hold, or terminate your participation if:
• the Program becomes medically inappropriate or unsafe
• you experience concerning side effects requiring discontinuation
• you fail to complete required check-ins or provide needed information
• you engage in abusive, threatening, fraudulent, or inappropriate behavior
Termination does not entitle you to a refund for paid membership fees, ordered medications, or completed
services.

14. Limitation of Liability
To the fullest extent permitted by law, we are not liable for indirect or consequential damages related to
Program participation, including delays, individual response variation, or failure to follow instructions. Our
total liability for any claim is limited to amounts paid for the specific Program period giving rise to the claim,
to the extent permitted by law.

15. Governing Law
These GLP-1 Terms are governed by Texas law.
